Why Behind the Thinking #
[The Operational Delusion]’s red failure question is “What’s really going on?” — the story the operator is telling himself about the operation and the reality the operation is living are often different. The spoke exists because operators default to the comfortable narrative rather than the accurate one; naming the delusion as a distinct filter spoke forces the operator to check the story against the reality before a decision clears the filter.
